no longer clean up env table

I made a change in the CVS code the other day to no longer clean up
the env table before each request.  This is old code that had been
borrowed a few years ago, and it no longer seems necessary.
Furthermore, it prevents things like the unique-id module from
functioning correctly, so it was worth changing.

I will be releasing a new version shortly.
