reintegrating neh branch into trunk

The branch 'neh' (new exception handling) has been out a couple of weeks for
anyone wanting to test it.

The branch has the code for the new 'abort_page' and 'abort_code' commands.

I haven't seen further comments on it and I propose to reintegrate it. I'm
going to allow a few more days for last minute comments before writing the
documentation and merge it back into trunk
