Following is the comment taken from jira issue https://issues.apache.org/jira/browse/OFBIZ-5436.

Tried to regenerate this issue but issue didn't seems to reappear.
Step followed:-
1) Created a new order for DemoCustCompany
2) Added GC-001 to the Cart
3) Selected a style and amount and click "Add to Cart"
4) Clicked the "Submit" button without entering any information in the survey fields.

Here redirected to order entry page which was showing validation error having message:-
The question Sender's Name: requires a response.
The question Recipient's Name: requires a response.
The question Recipient's Email: requires a response
