Section 4.5.6 on the page listed above is in the only area on the page that notes you must run the cloud-setup-management script.

When I was setting up CloudStack with only a single Management Server, I skipped section 4.5.6 because it appears to only be applicable if you are setting up more than one Management Server.  As a result of not running the cloud-setup-management script listed in section 4.5.6, my CloudStack installation did not function.
