[I] feat(providers): support per-model protocol routing without models.dev [maka]
Astro-Han opened a new issue, #4948:

Command Code exposes one connection and model catalog, but requires non-Anthropic models to use `/provider/v1/chat/completions` and Claude models to use `/provider/v1/messages`. Calling Claude through Chat Completions returns HTTP 400; this was verified while reviewing #4943.
Maka already resolves runtime behavior per model and supports models.dev overrides and provider-specific protocol selection. Providers absent from models.dev need an explicit way to describe this routing through the same resolver.
Acceptance:
- One connection and credential can serve models using different supported protocols.
- Discovery and runtime selection agree on each model’s protocol.
- Adapter, endpoint and reasoning replay semantics resolve together through the existing runtime resolver.
- Regression coverage exercises Chat Completions and Anthropic Messages within one connection.
