AOO 4.2.0 r1658822 en and de localisation, Win7 64bit
Works last in AOO 4.0.1
Open a new Math doc
insert some stuff e.g. a+b
close the document or close Math using the corresponding x/X Symbol in upper
right corner
AOO crashes
No crash if you close the document using "File > Exit" or save it before.
Crash if you close a new and empty Math doc.
Same behavior if you open the formula editor in Writer (Insert > Object >
Formula) and close Writer without saving.

If I use this example and close the document (small x at the end of the menu
strip) or close the application (large x at the far right of the title bar),
I see the usual Save / Discard / Cancel message indicating that the document
has been modified and do I want to save my changes.
Are you saying that message did not appear in either case? Was the application
stuck (did not close) or did it do something else?
The nothing-happening stuck case is one I have seen with my Logitech Zone Touch
Mouse that is usually cured by restarting my Windows 10 system.

After a new installation of Win7 and fresh installed AOO 4.1.2 the problem is
gone.
So it seems to be a Windows problem ...
This issue can be closed.

Confirmed with all AOO version newer than 4.0.1 including 4.1.4 RC3

It came down to be a problem with "assistive technology" enabled in the
OpenOffice settings.
Pedro wrote to me:
"I have figured out the solution (and the cause). I had a tablet driver
installed (a Wacom Bamboo). Even though the tablet itself was not
connected, all tablet features were enabled.
When AOO (and LO) detects such a driver it doesn't allow the "Support
assistive technology tools (program
restart required)" option to be unchecked.
Removing the driver solved the problem."
The problem remains:
AOO crashes on systems using assistive technology under certain circumstances.

Yes, and uninstalled/reinstalled 4.1.4
Also tested with your AOO420m1(Build:9800) - Rev. 1808637
The same crash happens with the 3 procedures described by the OP.
It is something related to Windows 7 x64 and only on some machines but still it
is a regression in AOO because the crash does not occur in 4.0.1

Maybe some libraries Are different?
Maybe the equivalent to ldd command could bring some clarity what is going on?

@jmpoo: The defect(s) are not reproducible because you have not provided clear
and sufficient steps. When you provide steps required to consistently reproduce
the defect, be clear about what you mean by 'crashes' because this is a vague
term. For instance, does 'crashes' refer to all open instances of AOO closing
whenever you close the Math product?
Also identify what AOO product you are referring to as 'document' in your
defect report.

Maybe there are errors visible in the Windows event logs?

Any of the 3 procedures causes consistent crash on AOO from 4.1.2 to 4.1.4
(yes, I'm using your pre-RC3 builds)
Unfortunately Win7 x64 seems to behave differently from Win10 in many aspects
(I haven't tested Win7 x86)

I can not reproduce a crash following the given steps in comment #0 with AOO
4.1.4 pre-RC3 on Windows 10 64bit and on Windows 7 32bit.
Can someone give a step by step instruction?
(Pedro: There is no RC3 yet, did you test with pre-RC3?)

Tested on another Win 7 x64 machine and I can not replicate the bug either.
Also tested on Win XP x86 SP3 and Win 10 x64 and no problems.
Reinstalling Windows 7 is not an option.
