Unless you can reproduce this with a standard piece of code outside of the 
Apache web server, we cannot diagnose and fix it.

You state you are using ap_rputs() and ap_rflush(), but those are Apache calls, 
not Xalan calls, so you should consult some Apache documentation or post a 
question on an Apache server list.
