Sorry Dave for messing you around, not sure what your other Mirror requirements are, but at Apache we really need the 'Headers' and 'Footers'  in place as they contain information required.
Other lighttpd users have experienced similar problems, but they configured their way out of trouble and got the header/footer showing. 
Let me know when you got that in place. Thanks.
