Using the TreeProcessor (Centipede, based on Cocoon 2.0.1) or the default 
sitemap implementation (current Cocoon CVS), the 'build docs' target fails with 
a nasty StringIndexOutOfBoundsException when the attribute 'href' is left empty 
on a 'menu-item' element in src\documentation\book.xml
