Attaching an amended patch with more fixes. First fix initializes the 
entryIndex during refresh. Second fix is in addEntry and adds entry to 
entryIndex with portlet name as the key (rather than object itself).

>From my testing, this particular patch will allow for persistent addition and 
removal of portlets. Any new portlets are added to local-portlets.xreg (this 
file must exist or add/remove will fail). One remaining issue is that if local-
portlets.xreg is empty to begin with, adds/removals do not persist.
