I applied the patch locally, tests passed. I then moved the classes to a {{mom.kafka}} package to avoid mixing different MOMs in the {{.mom}} package. 

I also touched up the XML examples in your documentation. I also did some minor touches to the code.

I fully expect some AMQP appenders to get in the mix at some point in the future.

The code is now in Git master.
