I can reproduce it on W7 and W10 by "printing" on xps.
This reminds me of PDFBOX-3776 and PDFBOX-3729 but not really a duplicate. One difference is that your PDF is very simple so I created a reduced one. I will try to create some java code that doesn't use PDFBox to reproduce the error so that I can submit it to oracle.
Workaround for you: use the dpi parameter of PDFPageable. Set it to a good value, e.g. 300 or 600.
