Apache POI generated word file used to work fine before Microsoft 365 version
2108 ( Here is version list:

but then it suddenly started behave wrongly.
Issues we are facing are not limited to table of content not appearing, title
page image not showing up with the header image, formatting issue.
Can you please help us to resolve this issue as the current microsoft update
has happened organizationwise. Can you please fix it to work with the stated
version of Microsoft office 365?

You might be better off talking to Microsoft. If a doc file that loads ok in
one version of Word and doesn't load ok in another version of Word, then that
is more of Microsoft issue.
If you attach a sample file here, then someone might have a look.
Apache POI is a volunteer organisation and the software is provided as is.
