cvschangelog output - put <date> and <time> inside <file>

This change would require adding a new attribute to the cvschangelog task to
allow the ant users to choose the new format, so it would make the manual still
a bit thicker.
I see the change as a low priority, since you can almost certainly reformat the
XML generated by ant with a style sheet to make it fit your needs.
