The problem is (most likely) the limited support for inline-keeps that has been added.
keep-together="always" expands to
keep-together.within-page="always"
keep-together.within-column="always"
keep-together.within-line="always"

You actually only need to keep the content together within the page (possibly in the column), so you need to set those components explicitly, so that keep-together.within-line defaults to "auto".
