What should happen during starup:
 * Convert all headers of index files into in-memory metadata.
 * For non-existing index files - scan corresponding segments and re-create them.
 * For the last segment - create mutable mem-table and fill with with what there is in the segment file. Hash violation should indicate position of the last valid log entry.
