1.  This kind of thing usually comes from an application.  The instances of
".php" in your access log give us a strong suspect.
2.  You appear to be using PHP with Worker MPM.  Didn't you read the warnings?
3.  The first fix to "bug in an old version" is always to upgrade.  Even if 2.2
is not an option, there are more recent 2.0 versions offering bugfixes.
