SSVM and CPVM on Cloudstack 4.3
Hi,
I have SSVM and CPVM in "running" State.
When I am taking console of any of this, On console
CPVM displays : sd 2:0:0:0 rejecting i/o to offline device
cannot execute "/sbin/getty"
SSVM displays : sd 2:0:0:0 rejecting i/o to offline device
Logs are as below..

I have seen IO failures occur when the primary storage is degraded. This
can happen due to NFS issues or a lossy network.
See if everything is OK with your NFS and network.
