   remap_stats: convert to using TSHttpTxnPristineUrlGet and TSHttpTxnClienReqGet
   remove pre remap continuation
   also fixes and adds an autest for --post-remap-host
   Includes memory leak fix for hostname when the old post_remap hook wasn't hit.
