updated DTrace patch for httpd trunk

(Don't worry, it is not intended as a [PATCH] patch ;) )

Much of this is the uncommitted part of Theo's original patch.  Much
of the rest is accommodation for generalized hook probes in APR-Util.

Hopefully I will work more on this trunk support, but I need to play
with or otherwise understand more about Theo's original patch and make
sure I didn't regress anything before proceeding.  For example:

* on what platforms/architectures did it build/work
* what were the compile-time or other warnings
* what are some non-trivial DTrace examples using the probes, for
comparing behavior
