I'm going to remove bandwidth_mgmt_xml.config and all teh code related to this. If someone has a concern with this, please let me know. This is, as far as I can tell, a remnant from the old MIXed streaming media protocols, that were half plugins - half TS core implementations. IF/when we support bandwidth management properly we can revisit this, but right now, this is really dead code.

Please speak up if you wish the code to be preserved, and I'll leave that, but I'll still remove the config file support, since it's confusing.
