.doc Check Box file (without table), no labels, shrinks together
This is the actual .doc file I tested with. The previous submission I made was
the .odt file, but I mistakenly labeled it as .doc, I apologize. The labels
disappear like before, but since there is no table to hold them they shrink
together.

I was able to replicate the bug on my build. AOO 3.4.1 Build 9593, Windows 7
32-bit.
Here's what I did:
1. Created a new document in Writer
2. Created three checkbox fields using the Form Controls toolbar
3. Added unique labels for each checkbox
4. Saved the document as an .odt
5. Saved the document as a .doc
6. Closed OpenOffice
7. Opened OpenOffice with both files for comparison
I noticed that the .odt file looked fine, but the .doc file had checkboxes with
no labels. In addition, the checkboxes that were on the same line oriented
themselves so that they were very close together. This may be because there is
no table to separate them. I've attached the .odt and .doc file I used to show
the checkboxes getting closer together.

I followed the steps as outlined by Paul and was able to replicate this bug on
AOO400m3(Build:9702) - Rev. 1503704, Windows 7 Ultimate 64-bit (Version 6.1
Build 7601: Service Pack 1)

I am using a window 7 machine with OpenOffice 4.0.1 . I tried to duplicate the
issue but was unable to do it with which the labels dissapear. However, when
opening the .doc file , I did received and error message "Office has dectected
a problem with this file. Editing it might harm your computer"

I was able to replicate this bug in Writer 4.0.1 Build:9714 Rev. 1524958
running on Windows 8.1.
To replicate this bug:
1. Create a odt file containing checkboxes and labels.
2. Save file as odt, then immediately save it as .doc.
3. close Writer.
4. Re-open writer and open the newly created .doc file.
When I replicated this bug the checkboxes persisted but their labels
disappeared.

I was able to replicate in AOO401m5(Build:9714) - Rev. 1524958
Windows 7 Home Premium 64 bit following Kyle B steps in comment 10. The defect
did not replicate if I first closed the .odt file, reopened and then saved as
.doc.
