I've built 2.2.0 an 2.2.2 from source on a virtual I386 server which uses
Virtuozzo as virtualization platform. I got the same effect with both versions:
The build process succeeded with no errors. Then I start Apache with the default
configuration and my browser shows "It works". Then I enable the "/manual" alias
for the local documentation. When I call it with the browser there is no output,
blank page. The logfiles don't show errors, the access logfile says that the
content was delivered normally (GET with status 200 or 206). This is the same
with prefork and worker mpm. The 2.0.58 and other 2.0.x versions don't have the
problem.

Try EnableSendfile Off.
Confirm that you can run the same build okay on real hardware.
Try gathering an strace of the failed processing and seeing if the
virtualization support team can spot something that isn't handled correctly on
their side.

OK, "EnableSendfile off" did it. Virtuozzo's virtualization seems to be very
different from others which emulate an I386 in a "closed box". My Linux box uses
a proprietary virtual filesystem by Virtuozzo.
