Windows 8 64-bit 
OpenOffice 4.2.0 AOO420m1(Build:9800)  -  Rev. 1692551

My attempt to replicate the bug: 

1- I clicked on the the "Font Color" icon in the formatting toolbar.
2- I selected the color "blue 3".
3- I clicked on the the "Font Color" icon in the formatting toolbar.
4- I selected the color "blue 4".
5- I clicked on the the "Font Color" icon in the formatting toolbar.

I was not able to replicate the bug. 

Can you attache a screenshot that shows the "white squares" in in the last row
of the font color table? Also, please write how did you change the font color,
there are many different ways to change the font color. 

Finally, I did not find ant related reports in the database.
