STEPS TO REPRODUCE:
1. Install roller 4.0 with Sun glassfish v2 (AppServer 9.1 version)
2. Launch and IE broswer to access to roller server
3. Login > click Main Menu > New entry > Design > Templates

Log:
Fixed ROL-1475, image URLs were incorrect due to double calls to <s:url>. The problem is only visible if Roller is not installed at context root, that's why Allen and I didn't notice it on our sites.

Thanks for a quick look at the issue. I just go back to reproduce the problem and realize that I have missed some important steps to reproduce the problem, sorry for that. Below is the adequate steps to create the problem
- Login > create a weblog entry (if does not have one yet) > select New Entry > Design > Custome Theme
- Click to enable "I want to copy the template from ....."
- Click Update Theme button
-Click Design tab > Template
Some of the bitmap name icon was not appears such as
http://localhost:8080/roller/roller/images/page_white_gear.png
Attached is the snap shot for the icon missing problem
