I have same issue in open source cassandra 3.8, I have high blocked Native-transport-requests in nodetool tpstats. I also see higher client latencies on co-ordinator when the local latencies are much low and no network issues. I am not sure if the client latencies are related to this. I am currently using default of 128 threads.
