The problem is that you specified language="en" on page-sequence. The values of the language and script properties are used to control how font tables are selected. Here, you have basically said to FOP that it should use the English (en) font tables for all the document content, including Arabic. Since the English font tables do not cover Arabic joining behavior, you are getting what you asked for. I would suggest you not specify language, or if you must specify the top level as "en", then you need to add language="dflt" to each Arabic text block.

Also note that you place writing-mode on the root element. It does not apply to root, but does apply to page-sequence. See [1] for details.
