Document has a number of tables in it

Every time, and I mean every time I open this particular document, OO crashes
before I have chance to touch the keyboard.

I think there is an issue with OO and tables. Put to many tables in a document
and OO can't cope.

I can open the attached document without problems in "AOO411m6(Build:9775)  - 
Rev. 1617669 2014-08-13 09:06:54 Mi, 13 Aug 2014" and in a build from current
trunk, both on Windows 7. Therefore it is likely, that something in your
installation of AOO4.1.1 was wrong. Perhaps you had used an incompatible
extension, or something in your user profile was damaged?

I would test it with an administrative installation of a daily build of current
trunk. Ask on mailing list or forum, if you need assistance.

Uninstalled OO 4.1.1 and re-installed 4.0.0.
4.0.0 opens the document without problems
