[Bug 12380] Close the page to create form

This has been retested in the openoffice 3.3 version. Within testing it was
determined that when you create your form, you are able to save and reopen the
form to edit. To do this you would need to select the icon to turn the design
back on. Once the icon has been selected you can then edit your form that  you
are creating.

To create a form, you will need to open "openoffice writer", from there you can
use the icons to create what you are looking to create (ie: text box, push
button), to edit the contents double click and a pop up box eill display to
edit. 
If you select the design mode off button, then you are no longer able to edit
the form, to change it back to edit, you can select the design mode icon again
and your boxes are open to select and edit. If you save and reopen, you can
relect the design mode and it enables you to edit the form.
To create the form control, you will need to select view =>tool bars->form
controls and the dock will display with the icons.

With the form controls open you are able to view what you are creating.
