Added a new chapter and referenced it from the External APIs chapter. Also did substantial reformatting on the new chapter.

I suggest it be moved to it's own appendix and referenced by all the places that expose it (currently just Thrift and the Shell, eventually REST -- pending completion of HBASE-5417).
