response.addCookie() not functioning properly?

When running ImportAttribute.jsp (sample code from Ch.11 - Core Servlets and 
JSP by Marty Hall), the code works fine on Tomcat 4.1.18, but not on 4.1.24.  
Source code of associated files can be found on the book companion site.  I've 
tried repeatedly and couldn't figure out why the code fails to work in 4.1.24.
