Resolution: Won't Fix
Given that we've never encountered a PDF like this, I'm going to close this issue. Anybody with an actual PDF to attach is welcome to re-open it.

Good call - DO NOT support this.
There is a long history as to why this was introduced at the time and
there are no documents in the wild that ever used it.
