Log:
Previous commit changed too much - it uploaded few more fixes.
It (partially) fixed #156 - spaces in <init-param> - I haven't added trim(),
but changed web.xml to not include spaces.
This fix rolls back the proposed fix for ResponseImpl - the test program works
the same with or without fix, and the fix will disable flush() from included
files. If we can reproduce the reported problem ( #142 - included text showing
before including servlet test), we can re-submit the patch.
