I've made a start, but its late so I'll wait till morning before testing and activating it.

A couple of questions I've made a guess at but you might want to clarify :-

1. Do you still want existing steps from 'In Progress' to resolved/closed to remain as choices in addition to Ready for Review, or would you rather Review be the only choice (other than Stop Progress->open) ?
(In other words, currently even if the Review step is added, those with resolve/close perms can choose not to review and just close it, this might be the desired effect, I'm guessing. Choices, choices :) )

2. Who should be able to act as the reviewer ? In other words , change status to reviewed->accept&resolved or reviewed->denied&open, currently I've made the choice that only folks who can resolved/close issues can do the review step. (And I assumed you would want the reviewer to deny as well as accept!)

3. I'll double check , but  think I've made it that only the person working on the issue (who it is assigned to whilst in progress) can then choose to put it in for review.
