Why this is marked as spam

Hello,
Recently I have notice that some legitimate emails that are coming to me =
are marked as spam even though they still score below 5.0(spam score).
Here is the part of the header of such an email:
X-Filtered-With: renattach 1.2.2
X-RenAttach-Info: mode=3Dbadlist action=3Drename count=3D0
X-Spam-Checker-Version: SpamAssassin 3.0.1 (2004-10-22) on =
mail.mydomain.com
X-Spam-Level:=20
X-Spam-Status: No, score=3D-4.1 required=3D5.0 =
tests=3DALL_TRUSTED,AWL,BAYES_40,
 HTML_50_60,HTML_MESSAGE autolearn=3Dham version=3D3.0.1


It even says in autolearn=3Dham and they the subject is changed to SPAM =
just like all the spam

Are you sure it wasn't scanned twice?
