case sensitivity on windows

It would be nice if special care were taken on Windows when
Subversion tries to create a new file where an existing file
differs only in case, if the OS is Windows.  (It works fine as-is
on Interix when case sensitivity is enabled.)

Not just Windows, OS/X has a "defective" filesystem, too:
