[PR] Use short names for authentication_handlers [couchdb]

Move away from erlang terms and use simpler names them. For compatibility we still accept existing tuple forms. Entries that are not one of these are ignored.

This drops previously working `{Module, Function}` entries without any log message, see #6166.
