I have managed to finally get graphics working with FOP 0_13_0 standalone
using the following:

  <fo:display-graphic href="my.bmp"/>

I don't think this is the version that ships with cocoon however, so you would
have to change your classpath to use the newer version of cocoon, which may or
may not work.
