3.2 and trunk are affected, 3.1 is not.
Fix and add a test case to prevent further regressions.

Never mind... the user was having qmail add "with ESMPTA;" rather than "with
ESMTPA". There's no issue with Theo's change.
I added a test case to trunk anyway (r541441)... we were lacking one for auth'd
qmail connections.
