Why isn't this application starting?

I have apps sitting in the ACCEPTED status at length even though there
are many resources available on my cluster.

Here is the scheduler status, and the application attempt:

There is obviously space for this application's one container request
- I've verified that there are nodes in the cluster with 100GB or more
of free memory, and nothing in the scheduler seems to indicate why it
is not starting.

Here is the app arriving in the resourcemanager log... after this no
further messages about the app ever appear:

Any idea how I can diagnose this?
