One more commit to go (probably tomorrow) to get mpm_winnt to track thread
status

I have scoreboard basically working on Windows.  In the process, I have
cleaned up the API significantly which will affect all MPMs.  My plan is to
commit the changes that don't affect other MPMs followed by the changes that
impact all the MPMs.
