In the Installation Guide, section 4.5.3.3 there is a note to copy the vhd-util to /usr/lib64/cloud/common/scripts/vm/hypervisor/xenserver but in 4.1 there is no longer a directory with that name.  The corect directory is a /usr/share/cloudstack-common/scripts/vm/hypervisor/xenserver.
