I propose the following patch:

UploadWebRequest.java:

    @Override

    public WebRequest newMultipartWebRequest(Bytes maxsize) {

        try {

            MultipartRequest multipartRequest = new MultipartRequest(req, maxsize);

            multipartRequest.setRequestParameters(getRequestParameters()); // the difference is here

            return multipartRequest;

        } catch (FileUploadException e) {

            throw new WicketRuntimeException(e);

        }
