MD5 conflicts

Could I get a quick concensus to rename md5.h to ap_md5.h? This issue
has created conflicts for quite some time.

Sure, +1, seems harmless enough.  The NT makefiles will have to be updated...

Someone should make sure any place we have second-class references to
things (e.g., the list of functions for AIX DSO) that those are all up to
date, and that a "make depend" is clean.
