While converting ConceptMapper to docbkx, I noticed that some cross refs in the PDF output were missing (they were present and worked in the HTML output). Eventually traced this to what looks like a long standing bug in the Docbook XSL style sheets for PDF output. I've posted a bug report to the Docbook project to get this fixed. Meanwhile, this Jira covers implementing the fix as a local customization.
