   So, that case where it was messed up early may have been a pebkam (oops).  
   - We've deployed #5929 and #6507 onto a machine -- 6507 is finding a lot of assets in this state.
   - Vijay has replicated the problem's symptoms by commenting out https://github.com/apache/trafficserver/blob/b5692b1bd8b7fa217eddfbb2234b05dc44795c33/iocore/cache/CacheWrite.cc#L1259
