To Part 1): The actual bug is fixed, but I've seen that your particular example
here may produce one more line in the second column than in the first column
when balancing a column. I'll have to take a closer look at this. I've seen this
behaviour before but was never able to pinpoint it, yet. But at least, it's not
as ugly as the bug you've found here.

To Part 2): This is indeed a feature. You're seeing the effects of the widows
and orphans properties which default to "2".
