yet more requests for inclusion in 0.22

Hi Justin,

In addition to the final blessing on QPID-4682 and QPID-4683 from the other
mails, I have a couple more requests for inclusion:

Description: change to the External authentication provider to construct
SSL client auth based usernames in the same format as the C++ broker. Very
isolated, low risk, and adds a config option to restore the previous
behaviour anyway.

Description: ensure that using a 0-8/9/9-1 connection with a NoAck
subscription and receiving persistent messages from durable queues does not
leave orphan queue entries in the message store. Very isolated, low risk.
