Allow users to set the table name.

Allow users to choose whether or not to drop+create the table via the reporter or whether they're responsible for creating the table.

Allow users to configure insert/upsert/update. The default is "insert id, status, timestamp".

This and the earlier jdbc reporter introduce breaking changes and will only be applied to 3.x.
