Acrobat 3 and fox:outline

I've found that Acrobat 3 cannot see Bookmarks produced with fox:outline. 
Apparently Acrobat 3 requires a /Count member of the root outline object, 
which is consistent with the PDF 1.3 spec. Acrobat 4 handles the FOP output 
with no count just fine, but some of my clients have a large installed base 
of Acrobat 3. The following patch adds a /Count to the root object.
